How to Create a Waiver
Read up on the process to create a general signup form here.
On the Content step, scroll to the bottom and find the section labeled Waiver.
Toggle ON "Enable waiver collection"
Put in any liability requirements and additional info in the "Waiver Text" field.
IF the signup form is a Waiver, this will automatically add a few things to the signup form flow:
A selection for "Adult" vs "Adult w/ Minors" on the form
IF the user selects Adult w/ Minors, they will then be prompted for number of minors (1-10) and based on that selection, we'll dynamically show fields for First Name, Last Name, and Date of Birth for each minor.
On submit of the form, a second screen will show with the Waiver Text you added in settings, will list the minors from the previous step and require a signature.
How to Download and See Signed Waivers
For an individual persona
Navigate to customers > personas and find the persona you'd like to look at
Click on them and open the individual persona page
On the left of the screen, underneath the name and other identifiable information, will be a section titled "Last Waiver Signed" with a "View" button.
Note: This will not show if the persona has NOT signed a waiver before.
For all personas
Navigate to customers > personas and click on the more (...) menu
Click "Download Signatures"
How to Add Minor Waivers to the Form
You don't need to do anything manually! When waivers are added to a signup form, this creates an additional field on the signup form where users will have to specify whether they are an adult or adult with minors. Once they click the option with minors, they will be required to input the number of minors they are signing for and each minor's DOB and name.
On the actual waiver, AIQ will include additional terms as follows:
"In consideration of my child being allowed to take part in this Activity, I confirm my full acceptance of this Agreement on their behalf. I certify that I have the authority to act for the minor child and to make decisions regarding their participation."
On adult personas, relevant information on minor waivers will also be stored in Personas > Persona Detail > General > Minors.
No separate profile or signup will be created for minors and their info will only be included as part of the adult persona
